Gastro
When starting a GLP-1 therapy, digestive discomfort can be common. Our providers may prescribe supportive nutrients like B3 and B5 which may help your body adjust to your medication.
Muscle
Quick weight loss can also sometimes mean muscle loss. Our providers may prescribe additives like glycine and B12, which may aid in muscle repair as you lose weight.
Energy
As your body adjusts to slower digestion and a reduced calorie diet, you may feel less energetic. Our providers may prescribe additives like methionine, inositol & choline, which may support fat metabolism for consistent energy.

Zepbound® dosing schedule and titration overview
Finding the right Zepbound® dose is a gradual, guided process.
If you’re new to GLP-1/GIP treatment, your clinician will typically start you on a low introductory dose, often 2.5 mg once weekly. Beginning low helps your body adjust to the medication and can reduce the chance of side effects before any increase is considered. Your dosing schedule is tailored to you based on your medical history, how you feel, and the goals you’ve discussed with your care team.
At your first follow-up check-in, your clinician will review how you’re tolerating Zepbound®, any side effects you’ve noticed, changes in your weight, and how your appetite and energy are shifting. If you’re doing well and seeing steady progress on your current dose, it may be best to stay there until your results begin to level off.
Remember that meaningful progress can take time. For many patients, visible changes may take several weeks to a couple of months as the dose is gradually increased and the medication has time to work.

Zepbound® injections: frequently asked questions
Find clear, clinician-reviewed answers to the most common questions about Zepbound® GLP-1/GIP treatment with Fittlly.
How does tirzepatide work in the body?
Tirzepatide is a dual GIP and GLP-1 receptor agonist, which means it acts like two natural gut
hormones—GIP (glucose-dependent insulinotropic polypeptide) and GLP-1 (glucagon-like peptide-1)—that
help regulate blood sugar, appetite, and digestion.
By activating both pathways, tirzepatide can:
• Increase insulin release when blood sugar is elevated
• Reduce glucagon, the hormone that tells your liver to release stored sugar
• Slow how quickly the stomach empties, helping you feel fuller for longer after meals
• Lower appetite and cravings, which can naturally reduce how much you eat
Together, these effects can support better blood sugar control and medically supervised weight loss when
tirzepatide is used as part of a structured treatment plan with clinical monitoring, nutrition, and
lifestyle changes.
What is Zepbound® prescribed for?
Zepbound® (tirzepatide) is an FDA-approved GLP-1/GIP injection used for chronic weight management in
adults living with obesity, or in overweight adults who also have weight-related health conditions such
as high blood pressure, type 2 diabetes, sleep apnea, cardiovascular disease, or high cholesterol. It is
always used together with changes in nutrition, activity, and lifestyle—not as a stand-alone quick
fix.
Tirzepatide belongs to a class of medicines called GIP and GLP-1 receptor agonists. By activating both
receptors, Zepbound® can:
• increase insulin release when blood sugar levels are high
• reduce glucagon, the hormone that signals the liver to release stored sugar
• slow how quickly food leaves the stomach, helping you feel fuller for longer
• decrease appetite and cravings, which can support reduced calorie intake and weight loss over
time
At Fittlly, Zepbound® is prescribed only after a licensed clinician reviews your health history and
determines that it is a safe and appropriate option within a structured medical weight management plan.
How do I use Zepbound® injections safely for weight loss?
Zepbound® (tirzepatide) is taken as a subcutaneous (under the skin) injection once a week, after you’ve
been shown how to use it by your care team. Common injection sites include:
• the abdomen, at least 2 inches away from the belly button
• the front or outer part of the thigh
• the outer back of the upper arm (this area should be injected by someone else)
Key points to remember:
• Rotate injection sites with each dose, and avoid scars, moles, irritated skin, or bruises. Never
inject through clothing.
• Check the medication before use: the liquid in the pen should be clear and colorless. Do not use it if
it looks cloudy, discolored, or contains particles.
• Always use a new, sterile needle for each injection, and never share pens, needles, or syringes with
anyone else. Sharing devices can transmit infections.
• Use Zepbound® on the same day each week, at any time of day, with or without food, unless your
clinician gives you different instructions.
• Follow the step-by-step instructions provided with your Zepbound® pen and any training from your
clinician or pharmacist. If you’re unsure about your injection technique or have questions, contact your
healthcare provider before using the medication.
What kind of weight loss can I expect with Zepbound®?
Weight loss with Zepbound® (tirzepatide) varies from person to person, and no one can guarantee a
specific amount of weight you will lose. Your starting weight, health conditions, dose, lifestyle, and
consistency all influence your results.
In a 72-week clinical trial of Zepbound® 15 mg in adults without diabetes but with obesity or excess
weight plus a weight-related condition, patients lost on average about 20% of their body weight when
Zepbound® was combined with diet and exercise changes. In the same study, people who only received diet
and exercise support lost about 3.1% of their body weight.
Your own results may be higher or lower than these averages. At Fittlly, Zepbound® is always used as
part of a supervised medical weight management plan, with a focus on safe, sustainable progress over
time, not extreme or rapid weight loss at any cost.
How soon will I start to see results with Zepbound®?
The timeline for seeing results with Zepbound® (tirzepatide) is different for everyone. Some people
notice changes in appetite or portion sizes within the first few weeks, while for others visible weight
loss may take a month or more. Both patterns can be normal, especially while your dose is being slowly
increased.
What matters most is consistency: taking your injections exactly as prescribed, following your nutrition
and activity plan, and staying in touch with your care team. Your clinician will adjust your dose over
time based on how you’re responding and any side effects you experience.
As a general medical guideline, a safe rate of weight loss is often around 1–2 pounds per week (about
4–8 pounds per month), though your individual pace may be slower or faster depending on your starting
point, health conditions, and goals. With Zepbound®, Fittlly’s focus is on steady, sustainable progress,
not extreme or rushed weight loss.
Where is Zepbound® available through Fittlly?
Zepbound® (tirzepatide) injections are available to many patients across the United States, but access
can vary by state due to local telehealth and prescribing regulations. During your online eligibility
check, we’ll ask for your location and clearly show whether Fittlly can prescribe and ship Zepbound® to
your state.
If Zepbound® is not available where you live, your clinician may discuss alternative treatment options
that are safe and appropriate for your situation.
Who can qualify for Zepbound® treatment with Fittlly?
Eligibility for Zepbound® (tirzepatide) is always determined by a licensed clinician after a full review
of your medical history, current medications, and goals. Zepbound® is not suitable for everyone, and it
is only prescribed when it is medically appropriate and safe.
In general, patients considered for Zepbound® typically:
• are 18 years of age or older
• have a BMI of 27 or higher
• are not pregnant or breastfeeding
• do not have certain medical conditions, such as:
o a history of certain thyroid cancers or serious thyroid disease
o gastroparesis (significantly delayed stomach emptying)
o pancreatic cancer or pancreatitis
o MEN2 (multiple endocrine neoplasia type 2)
o some forms of inflammatory bowel disease
This is not a complete list of eligibility criteria. As part of the Fittlly process, you’ll complete an
online health screening with our telehealth clinicians. Based on this evaluation, they will decide
whether Zepbound® is an appropriate and safe option for you, or whether another treatment approach would
better fit your needs.
Do I need to follow a specific diet while I’m on Zepbound®?
There isn’t a strict “Zepbound® diet,” but what and how you eat will strongly influence how you feel on
treatment and how effective it is. Your doctor or dietitian may give you personalized recommendations,
and it’s important to follow their advice.
In general, a balanced way of eating and regular movement will support weight loss, help manage side
effects, and make Zepbound® work better for you. Many patients feel better when they:
• Limit heavy, greasy, fried, and high-sugar foods, which can worsen nausea or digestive discomfort
• Reduce alcohol intake, as it can affect blood sugar and digestion
• Stay well hydrated, by drinking enough water throughout the day
Your Fittlly care team can help you choose an eating pattern that fits your lifestyle, health, and goals
while working alongside your Zepbound® treatment.
What are the most common side effects of Zepbound®?
Like all prescription medicines, Zepbound® (tirzepatide) can cause side effects. Many are mild to
moderate and improve as your body adjusts to treatment, especially when the dose is increased
gradually.
Common side effects can include:
• nausea or vomiting
• diarrhea or constipation
• abdominal pain or discomfort
• bloating, burping, or indigestion
• decreased appetite
• headache, fatigue, or dizziness
Not everyone will experience these symptoms, and for many patients they become less frequent over
time.
Less common but more serious side effects may include:
• severe or persistent abdominal pain (which could signal pancreatitis)
• gallbladder issues, such as gallstones or inflammation
• signs of dehydration (very little urine, feeling faint, very dry mouth)
• changes in vision, especially in people with diabetes
• possible thyroid-related issues, including a lump in the neck, hoarseness, or difficulty
swallowing
• allergic reactions, such as rash, swelling, or trouble breathing
If you notice severe, sudden, or worrying symptoms, seek medical help right away and contact your
healthcare provider.
Do not change your dose or stop Zepbound® on your own. If you’re concerned about side effects, speak
with your Fittlly clinician or your primary care provider. They can review your symptoms, adjust your
treatment if needed, and help you manage side effects safely. Always refer to the medication guide and
official prescribing information for the full list of potential risks and side effects.
What should I do if I miss a Zepbound® dose?
If you forget to take a scheduled Zepbound® injection, you can usually take the missed dose as soon as
you remember as long as no more than 4 days have passed since the day you were supposed to
inject.
If more than 4 days have gone by, skip the missed dose and wait until your next scheduled injection.
Then continue with your regular once-weekly schedule. Do not inject two doses close together or take a
“double dose” to make up for the one you missed.
If you’ve missed more than one dose, or you’re unsure what to do, contact your Fittlly clinician or your
primary healthcare provider. They can advise you on the safest way to restart, delay, or adjust your
dosing schedule.
How should I store and safely dispose of my Zepbound® injections?
To keep your Zepbound® (tirzepatide) treatment safe and effective, it’s important to store and dispose
of your pens, needles, and medication correctly.
Storage:
• Keep all medicine pens, vials, syringes, and needles out of reach of children and pets.
• Store your medication away from direct light and heat, following the instructions on the package and
pharmacy label.
• Refrigerate unused Zepbound® between 36°F and 46°F (2°C–8°C), and avoid placing it directly next to
the cooling element.
• Do not freeze Zepbound®. If the medication has been frozen at any point, do not use it—dispose of it
safely.
• Always check the expiration date or beyond-use date and do not use medication that is out of
date.
Disposal:
• Used pens and needles should go into an approved sharps container or a sturdy, puncture-resistant
container, according to your local regulations.
• Do not throw loose needles, syringes, or pens directly into household trash, and do not flush
medications down the toilet or pour them into the sink unless you are specifically instructed to do
so.
• Whenever possible, use a medicine take-back program or follow local guidelines for disposing of unused
or expired medication. Your pharmacist or local waste authority can help you find safe disposal options
in your area.
If you are unsure how to store or dispose of your Zepbound® safely, ask your pharmacist or
clinician—they can give you guidance tailored to your prescription and location.
